Welcome families of incoming Kindergarten students to the 2025-26 school year!  

Thank you to those who could join us for our March K Connect event! We hope it was informative and your incoming kinder enjoyed the music and classroom activity! For families who were unable to attend the event on the 12th, here is a copy of the presentation:

ENROLLMENT PROCESS/CHECKLIST

Before you enroll confirm your neighborhood school - go to and enter your address in School Finder.

Gather the following documents together to complete the registration process for your child:

*The Registration Form, Immunization Form, and Vision and Dental Forms can be found in all our supported languages by visiting the Enrollment & Transfer Website: pps.net/enrollment. (scroll down to 'Documents').

• Proof of Age Document - Birth Certificate, Passport or similar document

• 2 Current Address Verification Documents- Examples include a utility bill, cell phone bill, rental agreement, bank statement, insurance statement, or mortgage closing papers. Documents must be recent (within 3 months )and clearly show the date, parent/guardian name. and address. **Envelopes are not acceptable, please provide the statement page- you can redact any confidential information.

• Immunization Records- MESD Immunization Form Please transfer your records to the attached MESD form, and sign and date it before submitting. Be sure that you are entering vaccination dates chronologically from ther earliest dose date to the most recent dose date.  *Information on vaccine requirements by grade level can be found on the MESD website:

• Vision & Dental Screening Form- Vision & Dental Form 

• Individualized education (IEP) or special services (504) plan if applicable

HOW TO REGISTER ONLINE

 Go to  

• Create an online registration account

• Upload your address, immunization, and vision & dental screening documents

• Complete the online registration and click submit

• The school secretary will connect with the enrolling parent/legal guardian to follow up and confirm proof of age as well as gather any missing documents that were not uploaded online.

PAPER REGISTRATION  

If you are unable to register online, please complete a paper Registration Form and turn in to your school secretary with all supporting documents.
